BRIGHTPATH MEDICAL TRANSPORT

Transportation Service Agreement

Effective Date: 6-19-2026

This Transportation Service Agreement (“Agreement”) is entered into between Brightpath Medical Transport (“Company”) and the undersigned Client, Responsible Party, Family Member, Caregiver, or Authorized Representative (“Client”).

By scheduling transportation services with Brightpath Medical Transport, the Client acknowledges and agrees to the following terms and conditions.

 

1. SERVICES PROVIDED

Brightpath Medical Transport provides non-emergency, private-pay medical transportation services for ambulatory passengers.

Transportation services may include:

  • Medical appointments

  • Dialysis appointments

  • Hospital discharge transportation

  • Rehabilitation appointments

  • Senior transportation

  • Veteran transportation

  • Other pre-approved transportation services

Brightpath Medical Transport is not an ambulance service and does not provide emergency medical care.

 

2. NON-EMERGENCY DISCLAIMER

Brightpath Medical Transport is a non-emergency transportation provider.

The Company does not provide:

  • Emergency medical treatment

  • Medical monitoring

  • Nursing services

  • Medication administration

  • Physical assistance beyond normal transportation support

If a medical emergency occurs before, during, or after transportation services, Brightpath Medical Transport may contact 911 and local emergency responders.

 

3. CLIENT ELIGIBILITY

To receive transportation services, clients must:

  • Be medically stable for transport

  • Be able to safely enter and exit the vehicle with minimal assistance

  • Follow safety instructions provided by the driver

  • Wear a seatbelt when required by law

The Company reserves the right to refuse service if transportation would create an unsafe condition for the client, driver, or public.

 

4. SCHEDULING APPOINTMENTS

Transportation requests should be scheduled as far in advance as possible.

Brightpath Medical Transport will make reasonable efforts to accommodate requested dates and times; however, scheduling is subject to vehicle availability, traffic conditions, weather conditions, and operational considerations.

Transportation appointments are not guaranteed until confirmed by Brightpath Medical Transport.

 

5. PRICING AND FEES

Unless otherwise agreed in writing:

One-Time Transportation Services

  • Base Pickup Fee: $65.00

  • Mileage Charge: $3.00 per mile

  • Additional service fees may apply

Pricing may be updated periodically. Current pricing is available upon request.

 

6. WAIT-TIME POLICY

Brightpath Medical Transport includes up to fifteen (15) minutes of complimentary wait time after arrival.

After the initial fifteen-minute period:

  • Wait Time Fee: $15.00 per additional 15-minute increment

Wait-time charges may be added to the final invoice.

 

7. CANCELLATION POLICY

Clients may cancel transportation services without penalty when notice is provided at least twenty-four (24) hours before the scheduled pickup time.

Cancellations occurring less than twenty-four (24) hours before pickup may be subject to cancellation fees.

Same-day cancellations may result in charges up to the scheduled base fare.

 

8. NO-SHOW POLICY

A client will be considered a “No-Show” when:

  • The driver arrives at the scheduled location;

  • The driver waits fifteen (15) minutes after the scheduled pickup time; and

  • The client cannot be located or fails to board the vehicle.

No-Show charges may equal the full base transportation fee.

 

9. PAYMENT TERMS

Payment is due before or at the time of service unless alternative arrangements have been approved.

Accepted payment methods may include:

  • Credit Cards

  • Debit Cards

  • Square Invoices

  • Other approved payment methods

Outstanding balances may result in suspension of future transportation services.

 

10. CLIENT RESPONSIBILITIES

The Client agrees to:

  • Provide accurate scheduling information

  • Notify Brightpath Medical Transport of any changes

  • Be ready at the scheduled pickup time

  • Maintain respectful behavior toward drivers and staff

  • Comply with all safety requirements

Threatening, abusive, or unsafe behavior may result in immediate termination of services.

 

11. DRIVER RESPONSIBILITIES

Brightpath Medical Transport drivers will:

  • Operate vehicles safely and professionally

  • Arrive as scheduled whenever reasonably possible

  • Maintain a respectful and courteous environment

  • Follow all applicable traffic and transportation laws

 

12. SAFETY POLICY

For the safety of all passengers:

  • Seatbelts must be worn at all times when required by law.

  • Clients may not smoke, vape, or use illegal substances in company vehicles.

  • Drivers may refuse service if conditions create an unsafe transportation environment.

 

13. DELAYS AND UNFORESEEN CIRCUMSTANCES

Brightpath Medical Transport is not responsible for delays caused by:

  • Traffic congestion

  • Road closures

  • Weather conditions

  • Vehicle accidents

  • Mechanical failures

  • Government actions

  • Other events beyond reasonable control

The Company will make reasonable efforts to communicate delays as quickly as possible.

 

14. LIMITATION OF LIABILITY

To the fullest extent permitted by law, Brightpath Medical Transport shall not be liable for indirect, incidental, special, or consequential damages arising from transportation services.

Nothing in this Agreement shall limit liability where prohibited by applicable law.

 

15. PRIVACY AND CONFIDENTIALITY

Brightpath Medical Transport respects the privacy of all clients.

Personal information provided for scheduling, billing, and transportation purposes will be handled confidentially and used only for legitimate business purposes.

 

16. RIGHT TO REFUSE OR TERMINATE SERVICE

Brightpath Medical Transport reserves the right to refuse or discontinue transportation services when:

  • Safety concerns exist;

  • Fraudulent information is provided;

  • Payment obligations are not met;

  • Client conduct places drivers, passengers, or the public at risk.
