Brightpath Medical Transport
Membership Agreement
 

Effective Date: 6/19/26
 

This Membership Agreement (“Agreement”) is entered into between Brightpath Medical Transport LLC (“Brightpath”) and the Member (“Client”).

By enrolling in a Brightpath Membership Plan, the Client agrees to the following terms and conditions.

 

1. Membership Purpose
Brightpath Medical Transport provides non-emergency transportation services for private-pay clients requiring transportation to and from:

  • Medical appointments

  • Dialysis treatments

  • Physical therapy

  • Hospital discharge transportation

  • Senior living facilities

  • Rehabilitation centers

  • Personal errands (if included in plan)

Membership plans are designed to provide reliable transportation at a predictable monthly cost.

 

2. Membership Plans

Membership plans are billed monthly and may include:

Basic Plan

  • Limited monthly rides

  • Scheduled transportation only

Standard Plan

  • Increased monthly ride allowance

  • Priority scheduling

Premium Plan

  • Highest ride allowance

  • Priority scheduling

  • Preferred member benefits

Membership details and pricing are subject to change with advance notice.

 

3. Scheduling Rides

Members must schedule rides in advance.

Brightpath requests:

  • 24-hour notice for standard appointments

  • 48-hour notice for recurring appointments when possible

Ride availability is based on scheduling capacity and service area.

 

4. Payment Authorization

By enrolling in a Membership Plan, the Member authorizes Brightpath Medical Transport to charge the payment method on file for recurring monthly membership fees.

Payments may be made through:

  • Credit Card

  • Debit Card

  • Approved electronic payment methods

Failure to maintain an active payment method may result in suspension of services.

 

5. Cancellation of Membership

Membership may be canceled at any time by providing written notice.

Cancellation requests must be received at least:

7 days before the next billing cycle.

No refunds will be issued for partially used membership periods.

 

6. Missed Appointments & No-Shows

Members are responsible for being ready at the scheduled pickup time.

A ride may be considered a No-Show when:

  • The client cannot be located.

  • The client is not ready within 15 minutes of arrival.

  • The client cancels after the driver’s arrival.

Repeated no-shows may result in suspension of membership privileges.

 

7. Service Area

Brightpath primarily serves:

  • Las Vegas

  • North Las Vegas

  • Henderson

  • Boulder City

Transportation outside of the normal service area may require additional fees.

 

8. Safety Requirements

For the safety of all passengers and drivers:

Members agree to:

  • Wear seat belts when applicable.

  • Follow driver instructions.

  • Treat drivers respectfully.

  • Avoid disruptive behavior.

Brightpath reserves the right to refuse service when safety concerns exist.

 

9. Limitation of Liability

Brightpath Medical Transport provides transportation services only.

Brightpath is not responsible for:

  • Medical treatment decisions

  • Appointment scheduling errors

  • Delays caused by weather

  • Road closures

  • Traffic accidents beyond reasonable control

 

10. Membership Changes

Brightpath reserves the right to:

  • Modify membership plans

  • Update pricing

  • Update policies

Members will receive reasonable notice before significant changes become effective.

 

11. Agreement Acceptance

By purchasing or enrolling in a Brightpath Membership Plan, the Member acknowledges that they have read, understood, and agreed to the terms of this Membership Agreement.
